HF 990 Expands Iowa's Medical Marijuana Program: What Patients Need to Know

Iowa's medical marijuana program continues to grow, and one of the biggest updates in 2026 is the implementation of House File 990 (HF 990). Signed into law by Governor Kim Reynolds on June 2, 2026, the new law will take effect on July 1, 2026, bringing several important changes that make the program more accessible.
The biggest changes are that patients no longer have to be Iowa residents to apply for an Iowa medical marijuana card, and the number of licensed dispensaries is doubling.
Here's what HF 990 means, who it affects, and what hasn't changed.
What Is HF 990?
HF 990 is a new Iowa law that updates several parts of the state's medical marijuana program. Two of the biggest changes are the removal of Iowa's residency requirement for patients and the expansion of the state's dispensary network.
Before July 1, 2026, applicants had to prove they were Iowa residents when applying for a medical marijuana registration card through Iowa Health and Human Services (HHS). Under the new law, that residency requirement has been removed, allowing eligible patients who live outside Iowa to apply for an Iowa medical marijuana card as long as they meet all other program requirements.
HF 990 also allows the number of licensed medical marijuana dispensaries in Iowa to increase from 5 to 10, improving access for patients as the state's medical marijuana program continues to grow.
However, the law does not immediately open new locations. Instead, it authorizes Iowa Health and Human Services to begin the licensing and approval process for additional dispensaries over time. This means new dispensaries will open gradually as the state reviews applications, selects providers, and approves new operating locations.
As these new dispensaries are licensed and become operational, patients can expect improved access, shorter travel distances, and more purchasing options across Iowa.
Out-of-State Patients Can Now Apply
The most significant change under HF 990 is that Iowa's medical marijuana program is now open to qualifying patients regardless of where they live.

You Still Need a Licensed Iowa Marijuana Doctor
Although out-of-state residents can now apply, one important rule has not changed.
Every patient must still receive certification from a licensed Iowa marijuana doctor. If your marijuana doctor is licensed only in another state, they cannot certify you for Iowa's medical marijuana program.

Medical Marijuana Cards Are Still Required

HF 990 also continues Iowa's reduced registration fee program for eligible patients. The standard state registration fee is $100, but certain patients can pay a reduced fee of $25.
Patients may qualify for the reduced fee by providing documentation showing they receive one of the following benefits:
Social Security Disability Insurance (SSDI)
Supplemental Security Income (SSI)
Iowa Medicaid
Hawk-I
Eligible out-of-state patients may also receive the reduced registration fee if they provide documentation showing they participate in one of these qualifying assistance programs.
More Medical Marijuana Dispensaries Are Coming to Iowa
HF 990 doesn't just expand patient eligibility; it also lays the groundwork for improving access to medical marijuana across the state by doubling the number of licensed dispensaries in Iowa.
Until now, Iowa law allowed for up to five dispensary locations statewide, meaning many patients have had to travel long distances to purchase their medication. Under HF 990, the state can now license up to 5 additional dispensaries, which will make it easier for patients to access medical marijuana closer to home.
