How to Apply for Your Iowa Medical Marijuana Card

Getting your Iowa medical marijuana card is a simple process. Follow the steps below to complete your application and begin your journey toward relief.

Step 1: Meet With a Healthcare Practitioner

Schedule an appointment with us and meet with one of our certified healthcare providers. Our team will guide you through the evaluation process, answer your questions, and ensure your Healthcare Practitioner Certification Form is signed so you can move forward with confidence.

Step 2: Complete the Patient Application

Once your certification form is signed, submit your Adult Patient Application online using the state’s patient application portal.


If you’re unable to apply online, contact the Iowa Department of Health and Human Services at medical.cannabis@hhs.iowa.gov or call 515-725-2076 for assistance.

Step 3: Include Required Documents

Every application must include:

  • Your Healthcare Practitioner Certification Form (signed by your healthcare practitioner)

  • A completed Adult Patient Application (paper or online)

  • A copy of your photo ID, such as:

    • A valid Iowa driver’s license

    • A valid Iowa non-operator ID card

 

Step 4: Submit Payment

The standard application fee is $100. However, you may qualify for the reduced fee of $25 by submitting proof of one of the following:

  • Social Security Disability Benefit (SSDI) notice

  • Supplemental Security Income (SSI) payment receipt

  • Iowa Medicaid member card

  • Hawk-I member card

 

Step 5: Wait for Approval

Once your application is reviewed:

  • If you provided an email, you will receive a temporary card via email, and your permanent card will be mailed to you.

  • If you did not provide an email, your permanent card will be mailed to the address listed on your application.
