No. You cannot smoke marijuana in Iowa, even if you have a valid medical marijuana card.

Can You Buy Marijuana Flower in Iowa?

The Iowa Department of Health and Human Services specifically states that flower, including loose leaf and joints, is not legal for registered patients or caregivers to possess.
Iowa's Medical Cannabidiol Board reaffirmed its position against allowing raw marijuana in the state's medical program in its 2025 recommendations to the Iowa General Assembly.

Can You Vape Medical Marijuana in Iowa?
